The Home-Based Acute Care Occupational Therapist is responsible for therapy evaluations, treatment planning, implementation, and discharge planning for inpatient hospital at home patients in the home and hospital setting in accordance with established policies while maintaining the highest degree of quality patient care.

Required Qualifications:

• Education sufficient for licensure
• Upon hire: Washington Occupational Therapist License Or
• Upon hire: Washington Occupational Therapist Limited Permit License Or
• Upon hire: Washington Occupational Therapist Temporary License
• Within 30 days of hire: National Provider BLS - American Heart Association
• Upon request: Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. See policy for additional information.
• 1 year of Therapy experience with adult geriatric and/or individuals with physical disabilities

Preferred qualifications:

• 1 year of Therapy experience in home care or in related rehabilitation setting
• Experience with the geriatric population

About the Team

Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.

HCC provides a full range of services and support for people of all ages, especially those facing chronic or life-limiting illnesses.
