THE ROLE

Under the direction of the Chief of Infusion and Home Medical Equipment, the Director for Pharmacy Quality, Medication Safety, Regulatory Compliance, and Process Improvement has responsibility for providing direct caregiver supervision and leadership oversight for a regionalized multi-state clinical excellence and quality program for Providence Infusion and Pharmacy Services and Home Medical Equipment ministries.

The Director for Pharmacy Quality, Medication Safety, Regulatory Compliance, and Process Improvement has direct oversight responsibility and leads both the Quality Pharmacist Manager and Quality Nursing Manager. Pharmacy Technicians with specialized quality functions and focus are part of the extended Quality and Safety team. The Director is responsible for coaching all ministry leaders and caregivers to achieve top quality outcomes, as prioritized in the quality plan to performance at or comparable to the actual or estimated 90th percentile (or higher as appropriate) and improve patient safety, prevent avoidable patient harm, thereby mitigating risk to the organization.

This individual provides a major resource to the ministries as it relates to medication and equipment safety, regulatory and accreditation compliance, and performance improvement.

Through the ongoing collection and analysis of data and in collaboration with Home and Community Care and regional/local leadership the position identifies opportunities improvement and leads sustained excellence in quality and safety outcomes. When approved as priorities the position facilitates Continuous Quality Improvement efforts to address opportunities by developing and deploying systems that will enhance results. This includes facilitating processes to assess, analyze, and trend data and conduct research and implement evidence based best practice to improve results. The position is responsible for collaborating with leaders and Home and Community Care quality/safety caregivers and to engage front line caregivers in evidence based best practice transitions to improve participant safety results.

In collaboration with the Providence Infusion and Pharmacy Services and Home Medical Equipment key leaders across multi-state operations and service areas, this position is responsible for developing, implementing and monitoring safety, quality, compliance and risk programs. The position is accountable for the quality/safety plan to assure services provided are in compliance with applicable regulatory and accrediting bodies including but not limited to The Joint Commission, Regional and Statewide Department of Health agencies, State-specific Board of Pharmacy regulations, Drug Enforcement Agency, and Centers for Medicare/Medicaid Services as they apply to the Home Infusion Pharmacy Therapy Services (and related services such as direct nursing and direct nutrition services), Long Term Care Pharmacy, Pharmacy Manufacturing and Home Medical Equipment.

This position has primary responsibility for The Joint Commission survey preparedness in collaboration with local ministry leadership and Home and Community Care leadership with a goal of constant and ongoing accreditation readiness.

Work with ministry leaders and support personnel, assuring safe, effective, and efficient use of all Ministry IS systems involved in the provision of patient care including home infusion software, QS1, EPIC and that data is used to measure and monitor compliance with best practices.

The position collaborates with local and Quality leaders and coordinates and facilitates process improvement efforts related to improving patient outcome and satisfaction. Provides a proactive approach to meet patient expectations by developing and deploying processes that are anticipatory and that interact with and engage patients and caregivers.

The position is responsible for training staff on service excellence skills and methods.

This position serves as a service line safety officer.

ESSENTIAL FUNCTIONS

  • Works cooperatively with ministry leadership and all departments across multiple states in directing quality improvement strategies and medication safety initiatives.

  • Lead and collaborate with leaders as needed to develop and execute action plans for clinical and quality results on Quality Management Goal Report and Balanced Scorecard that are performing under target.

  • Design and provide leadership to deploy standardized auditing processes around critical clinical practices to support accountability, improvement and regulatory readiness.

  • Partner with providers across the continuum of care to improve patient outcomes in areas identified in the Clinical Strategic Plan.

  • Provides direction and oversight to quality and safety team and collaborates and coaches ministry leaders in activities to achieve performance improvement and quality standards for regulatory, accrediting and licensing organizations.

  • Lead regulatory and accreditation survey preparedness efforts in collaboration with local leadership and Home and Community Care to achieve a state of constant readiness.

  • Provide leadership and oversight for emergency and disaster preparedness and planning for each service line or ministry with focus on alignment wherever possible.

  • Work with managers and analysts to use data from clinical systems to measure and monitor compliance with best practice implementation across the service line and at the local and individual level.

  • Assists in the development and orientation of new caregivers, as relates to safety, quality, and compliance in cooperation with the responsible leader(s).

  • Conducts target medication quality and safety programs or other efforts designed to optimize the quality and safety in the use of pharmaceuticals and equipment. Participates and provides guidance for studies on medication safety and quality with all Ministry departments and initiatives.

  • Continuously reviews pertinent quality and safety alerts as relates to medication management (e.g., TJC Sentinel Event Alerts, ISMP Safety Alerts, FDA MedWatch) and designs appropriate action for the organization, in cooperation with the responsible leader(s)

  • Systematically monitors/provides oversight of incident reports and reported near misses for the Ministry. Collaboratively investigates near misses and other incidents to help identify potential causes and process design improvements.

  • Collects, monitors, analyzes and provides guidance to established quality metrics, including adverse drug events, medication errors/accuracy, patient satisfaction, and other metrics, providing quarterly reports to service lines in a prompt and timely manner.

  • Leads a team and supports prioritized ministry performance improvement initiatives. Measures and documents effectiveness of the improvements.

  • Collaborates with ministry leadership on policy and procedure changes that will improve quality and safety practices.

  • Lead ministry-based patient satisfaction and safety committees.

  • Empowers others by recognizing and developing the unique talent of others to maximize their contribution. Assists leaders and caregivers in adjusting to continuous change and to projects/tasks that support professional growth. Delegates authority commensurate with responsibility for special projects/tasks.

QUALIFICATIONS

  • Bachelor's Degree in Pharmacy; or PharmD (Doctor of Pharmacy)

  • Master's Degree in Health Administration or health related field (preferred)

  • Upon hire: Certification with Operational Excellence as a Change Facilitator or Green Belt (training may be obtained once employed) (preferred)

  • Training in Operational Excellence or equivalent Quality Improvement process methodology (preferred)

  • 5 years of pharmacy experience, or 3 years of experience with an advanced degree or residency

  • Project management or related management/facilitation experience

  • Experience in management of people and processes and working in a collaborative environment with operational leaders.

  • Medication, supply and equipment use management experience (preferred)

  • Experience in organizing, evaluating and conducting medication safety evaluations and policy management (preferred)

  • Experience in customer service and customer advocacy (preferred)

  • Knowledge of Pharmacy, Home Health, Medical Equipment, CMS regulations/standards, JCAHO standards, and other applicable professional practice standards

  • Knowledge of regulatory agencies across multiple states (Departments of Health, Boards of Pharmacy, Centers for Medicare/Medicaid Services, Drug Enforcement Agency and The Joint Commission)

  • Strategic thinking skills regarding medication safety, quality, process improvement, compliance, risk and safety

  • Knowledge of risk management processes, risk mitigation, sound decision making and appropriate utilization of external resources

  • Knowledge of Joint Commission regulations and survey process

  • Skilled in continuous quality improvement tools and methods

  • Must have effective oral/ written communication skills to perform job.

  • Strong analytic skills

  • Excellent interpersonal skills and the ability to work with a wide variety of people.

  • Effective written, verbal communication and presentation skills

  • Proficiency with MS Office products

  • Position requires ability to travel (occasionally with little or no advanced warning).
