Care Manager RN in Utilization Review Unit at Providence St. Patrick Hospital, Missoula, MT

The Nurse Case Manager (NCM) is responsible to manage a caseload of patients across the continuum of healthcare services and assure optimal quality outcomes, patient satisfaction, and appropriate use of resources. The NCM is responsible for directing an organized, patient-centered interdisciplinary team system that coordinates the delivery of healthcare services by continually monitoring, evaluating, and modifying the treatment plan to achieve optimal patient outcomes and efficacious use of resources. The NCM coordinates with physicians, Case Management Social Worker, and other health team members to expedite medically appropriate, cost-effective care and promote quality through evidence based medical practice throughout the health care system.

Shift details: This is a part-time, 0.8 FTE (32 working hours), variable shift position.

Required Qualifications:

  • Graduate of an accredited school of nursing Or

  • Graduate of an accredited health information technology program.

  • Upon hire: Montana Registered Nurse License

  • 3 years of experience in Clinical Nursing

  • Hospital case management experience

Preferred qualifications:

  • Upon hire: Current Case Manager Certification
