Description

Providence at Home is a growing division of Providence Life Services, which provides a continuum of healthcare, focused on senior living. Providence at Home is seeking a Home Health Registered Nurse (RN), Full Time in Lansing, IL and various zoned locations throughout the south suburbs. Providence at Home is a Joint Commission accredited and celebrating 30 years of home-based services.

Schedule Available: Monday - Friday during normal business hours. On-call duties receive additional compensation.

Summary of essential job functions:

  • Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(s).
  • Regularly re-evaluates patient nursing needs.
  • Initiates the plan of care and makes necessary revisions as patient status and needs change.
  • Uses health assessment data to determine nursing diagnosis.
  • Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
  • Initiates appropriate preventive and rehabilitative nursing procedures.

Requirements

  • Must be a Licensed Registered Nurse (RN) for the State of IL.
  • Minimum of 1-2 years’ experience, at least one of which in the area of home care nursing, hospice, ER or Med surg.
  • Must be CPR certified.
