Description The clinical patient navigator for valvular heart disease serves as a consistent resource to support optimal care delivery for cardiac valve patients and other projects as assigned, focusing on identification of patients who can potentially benefit from cardiac valve procedures based on cardiac echocardiogram findings and symptomatology, determine if patient is receiving optimal medical treatment if not a surgical candidate, reaching out to responsible provider with treatment recommendations as appropriate, and track patients disease progression. The navigator is knowledgeable of multiple treatment decisions, cardiac evidence-based standards, best practice guidelines and maintains care collaboratively across the continuum.
