Nursing Process: Assessment: ◦ Contributes to the assessment by completing the appropriate section of Nursing admission documents and orients patient to room and unit routine on admission. ◦ Continually collects patient's signs and symptoms, through observation and interview, gathering information and reporting patient's needs, problems and changes in condition to the RN in a timely manner. Planning: ◦ Participates in change of shift report ◦ With supervising RN, plans and organizes nursing care for a specific group of patients, using the appropriate resources. ◦ Contributes to development of an on-going plan of care based on the information gathered, including patient teaching and discharge planning needs. Implementation: ◦ Provides safe and appropriate bedside nursing care to a specific group of patients. ◦ Manages patient assignment effectively and assist co-workers as the need arises. ◦ Maintains an orderly appearance of the patient's room and unit work area. ◦ Supports the patient’s right to safety, privacy and confidentiality. ◦ Administers approved (see addendum) medications according to hospital policies and procedures. ◦ Recognizes precautions, interactions and side effects of medications, immediately reporting any unusual event to the RN and/or physician. ◦ Performs IV procedures according to hospital policies and procedures; after training and certification Evaluation: ◦ In collaboration with the RN, assists in making adjustments in the plan of care. ◦ Informs the RN and/or physician of the patient's response or lack or response to the nursing care provided. DOCUMENTATION: Accurately and legibly documents data, nursing care treatments and medication on appropriate forms, per hospital policy. TEACHING: Patient and Family: Participates in patient/family teaching and discharge instructions as determined by the plan of care. Staff: Participates in unit orientation of new staff, students, and outside nursing personnel. PROFESSIONALISM - Adheres to Swedish Edmonds Standards of Behavior: Appearance: Meets the standards of the printed grooming/dress code. Punctuality: Upholds hospital policies Attendance: Upholds hospital policies Participation: Actively participates, and by performance, shows an awareness of unit and organizational activities and initiatives. Flexibility: Willingly participates in changes of practice to enhance patient care and hospital performance. Cost Effectiveness: Optimizes resources, while delivering excellent care. Coping Skills: ◦ Effectively handles routine patient requirements and obtains necessary assistance. ◦ Deals with personal problems in a manner that does not interfere with delivery of patient care. Policy Adherence: Adheres to Swedish Edmonds general personnel and department policies. EDUCATION: In-service: Completes all mandatory assignments. Continuing Education: Is accountable for keeping nursing knowledge current and at community standard. COMMUNICATION: Patient and Family: Demonstrates effective verbal and non-verbal skills to promote a caring and nurturing atmosphere. Utilizes AIDET concept to support patient satisfaction. Staff: ◦ Utilizes proper channels for communication. Supports effective communication by using SBAR process. ◦ Effectively communicates needs of patient assignment. ◦ Promotes harmonious working relationships through use of direct, tactful and sensitive interaction. QUALITY IMPROVEMENT: Participates in unit based Quality Improvement Program, Organizational Quality Initiatives, and Core Measures, in an effort toward supporting the goal of improving patient care. Required qualifications: Graduate of an accredited school of practical nursing, including coursework in pharmacology. Upon hire: Washington Licensed Practical Nurse Upon hire: National Provider BLS - American Heart Association Recent experience in an acute care facility preferred, or related work experience.
