Description Diabetes Educator RN @ Swedish First Hill Part Time (20 Hour Week) Day Shift Please note: This position will provide education for both inpatients and outpatients with diabetes. Educates patients about diabetes self-management in individual or group settings, using approaches that are culturally relevant and age appropriate. Assesses the needs of patient and their family, formulates an educational plan, implements the plan, and evaluates and revises the plan as needed. Functions as a resource to the patient, the community, and other health care professionals both within and outside the Center. Coordinates ongoing patient care and support with appropriate members of the health care team. Provides education, training, and support related to diabetes technologies. Plans and conducts continuing education programs in diabetes for allied professionals and the general public. Participates in the assessment, planning, implementation, and evaluation of the diabetes education program at the Center, including continuous quality improvement and other activities related to maintaining recognition with the American Diabetes Association. Applies nursing process and education of patients and families including assessing the needs of patient and family, formulating an educational plan, implementation of the plan and evaluation and revisions as needed. Communicates information and coordinates patient care with appropriate members of the health care team. Demonstrates skills in a variety of educational approaches including one-on-one instruction, group classes and operation of audio-visual equipment. Maintains current knowledge in the field of diabetes including attendance of conferences and meetings, reading current literature and attending in-services and trainings. Maintains skills in the use of diabetes related technology and equipment. Acts as a resource person for other health care professionals, patients and members of the community. Assists in the medical management of patients, according to established policies and procedures. Also acts as an advocate on behalf of the patient and their ongoing medical care. Educates patients in both in-patient and outpatient settings. Coordinates participation in research programs and studies as assigned. Uses educational approaches, which are age and culture relevant. Required qualifications: Nursing degree/diploma. Bachelor's Degree Nursing degree (BSN) from an accredited School of Nursing. All Registered Nurses without a minimum of a Bachelor’s degree in Nursing must obtain a Bachelor’s degree in Nursing or higher within 6 years of hire. A RN with 20 years of RN experience will not be required to obtain a BSN Exceptions will be considered on a case-by-case basis at each facility by the most senior administrative nursing leader in consultation with the Human Resources leader. RNs employed prior to 1/1/13 are encouraged to obtain a Bachelor’s degree or higher degree in Nursing, but obtaining the degree will not be a condition of employment. Upon hire: Washington Registered Nurse License Upon hire: Certified Diabetes Care and Education Specialist (CDCES) (If eligible, must certify within 12 months of hire) 4 years Nursing experience, preferably one year in a community health or ambulatory care setting. One thousand hours of diabetes education experience in a variety of settings. Experience and comfort with teaching and public speaking. Preferred qualifications: If primary nursing experience is in a hospital setting, additional experience is desirable. Why Join Providence?
