Providence Health is Hiring a Clinical Pharmacy Specialist Near Medford, OR

Description

The Clinical Pharmacy Specialist (CPS) will collaborate with primary care physicians and staff to provide coordinated, comprehensive, high quality care to PMG patients. The responsibilities of the Clinical Pharmacy Specialist predominantly fall into the following areas: (1) direct patient care, (2) population-based disease management, (3) preceptor/educator, (4) pharmacy support, (5) research/projects.

Required qualifications:

  • PharmD From an accredited college or school of pharmacy.
  • Coursework/Training: Completion of a PGY-1 residency.
  • Coursework/Training: Completion of a specialty/PGY-2 residency in ambulatory care. Or equivalent educ/experience
  • 3 years' experience in primary care pharmacy (or related area) involving direct, collaborative patient care.
  • Upon hire: Oregon Pharmacist License
  • Within 2 years of hire National Certification from Board of Pharmacy Specialties
  • Upon hire: Washington Pharmacist License - based on clinic assignment
  • Experience in writing, including research protocols, medical papers for publication, and educational pieces for patients and healthcare providers.
  • Experience precepting students and/or residents.
  • Experience in writing, including research protocols, medical papers for publication, and educational pieces for patients and healthcare providers.
