Qualifications: must be active/eligible on IDPH website/ must have hands-on CPR card, and 1 full year of CNA experience within the last 3 years....
Agency requirements:

• TB and *Physical Health Statement* within last year
• record of MMR and Varicella immunizations within last 10 years.
• must be COVID vaccinated
• must show proof of HS Diploma or Transcripts...
