Job Description: Direct Support Professional

Reporting Supervisor: Program Director or designee

Duties:

o Provide clients with quality day habilitation experience, assisting with toileting, feeding, client activities, in areas as outlined by the client Individual Service Plan ( ISP)

  • Provide ongoing protective oversight to people attending the Day Habilitation Program.
  • Responsible for implementing the Day Habilitation Plan for each person following the safeguards stated in their ISP/IHP Plans.
  • Complete monthly group calendar activities based upon input from people supported.
  • Complete daily activity logs accurately and timely for people supported.
  • Enter information accurately and contemporaneously on each person’s billing sheets and/or electronic system, on a daily basis to ensure regulatory compliance.
  • Implement behavioral support plans a written and ensure associated data collection.
  • Successful training and demonstrated competency of DDD training provided on NJ College of Direct Support and program trainings.
  • Establish and maintain a rapport with community supports.
  • Establish and maintain a working relationships with the person’s circle of support.
  • Provides training, assistance and/or supervision to individual(s) in the areas of daily living, personal care, financial management, community and social integration, medication administration, and/or job skills development.
  • Establishes a therapeutic relationship with the individual receiving services to ensure effective guidance, support and service delivery. Ensures objectives are met in accordance with the Individual Service Plan.
  • Assist in maintaining a clean environment. Proper use and maintenance of equipment and appliances.
  • Mandatory attendance at all scheduled in-service trainings and staff meetings.
  • Drive agency vehicles safely to transport individuals to and from program, and to various volunteer sites/ community sites daily.

Qualifications:

o At minimum, a high school diploma or equivalent with 2 years of experience in health direct care/support is required

o Employee must cooperate with the officer licensee and department of Human Services staff in any inspection or investigation

o Employee must successfully complete and demonstrate proficiency in all areas of required training o Valid Driver’s License

o Must be at least 18 years of age or older

  • Knowledge of CCW Waiver Procedures or minimum 1 year of experience working with persons with Intellectual Development Disabilities or in another health related field
  • Good communication, writing and computer skills
  • Valid driver’s license
  • Ability to work independently
  • Good organizational skills
  • CPR/First Aid Certification

Job Type: Full-time

Pay: $18.00 - $19.00 per hour
