DUTIES:
--Responsible for residents’ care as assigned by Wellness Director and her Assistant as
detailed in the Daily Assignment Sheet and the Medication Administration Record.
--Follow through on, or communicate to the appropriate person, any requests made by
residents and/or their family members.
--Provide medication assistance with the approval of the resident and his/her
representative.
--Complete a Medication Assistance Training Program.
--Provide medication assistance following The Promenade of Chestnut Ridge’s policy and
Procedures.
--Perform with on-going proficiency, which may include ordering medication, picking up
MD orders, transferring MD orders, completing all documentation required.
--Record receipt of medications brought into the facility and medications sent out with
residents and their families.
--Ensure that all residents receive their medications as ordered and that they have a
sufficient supply of medication on hand so that residents never run out and follow
through with families to ensure same
--Report any refusals and/or reactions of medications to the Wellness Director and her
Assistant
--Follow written and verbal directions of the Wellness Director and her Assistant.
--Prioritize tasks to ensure optimum services to the residents as requests and needs
change.
--Communicate arising needs of residents to other management staff during a shift.
--Provide verbal support, encouragement, and motivation to residents to participate in
physical, mental and social activities.
--Answer calls from residents promptly and courteously.
--Be available during your shift in the Wellness Office to accept incoming calls and
problems unless specific tasks require you to be in another part of the building.
--Report and record all unusual reactions or changes in a resident‘s condition to other
Wellness staff through the Wellness Daily Log.
--Assist with providing a safe environment and report immediately any unsafe condition
to the Wellness Director.
--Participate in general overall upkeep of total area to assure a safe environment for staff
as well as residents.Clean and store equipment in the proper area, especially a daily
cleaning of med cart by night staff.
--Obtain vital signs and supervise recording of weights on a monthly and PRN basis, and
to record promptly.
--Demonstrate good infection control techniques, including hand washing.
--Complete assigned tasks before leaving premises.
--Report to incoming shift/med person before leaving your shift.
--Perform Narcotics Count with someone from the oncoming shift.Sign off before
leaving shift.
--Adhere to the Resident’s Bill of Rights.
--Perform any and all such duties related to the effective and efficient operation as may be
assigned.
