Promedica Total Rehab+ Moorestown is Hiring a LICENSED SOCIAL WORKER Near Moorestown, NJ
We are looking for a Social Worker to join our team and provide geriatric care to our elderly clients. The successful candidate will be responsible for assessing the needs of our clients, developing care plans, and providing emotional support and guidance. The ideal candidate will have experience working with the elderly, a strong understanding of geriatric care, and excellent communication and interpersonal skills.
Responsibilities:
- Provide psychosocial assessments of elderly clients.
- Develop and implement treatment plans for elderly clients.
- Provide counseling and support to elderly clients and their families.
- Conduct home visits to assess the living environment of elderly clients.
- Advocate for the rights of elderly clients in the community.
- Work with other professionals to coordinate services for elderly clients.
- Assist elderly clients in accessing community resources.
- Provide counseling and therapy services to individuals, families, and groups
- Conduct assessments and develop treatment plans based on client needs
- Collaborate with other professionals, such as psychologists and psychiatrists, to ensure comprehensive care
- Advocate for clients and provide support in navigating social service systems
- Assist clients in accessing community resources and services, such as child welfare programs, workforce development initiatives, and Section 8 housing
- Supervise and mentor social work interns or other staff members
- Utilize evidence-based practices, including motivational interviewing techniques, to facilitate positive change
- Must have a valid license as a Clinical Social Worker
- Experience working with diverse populations, including children and families
- Knowledge of child welfare systems and regulations
- Familiarity with Medicare guidelines and reimbursement processes
- Strong assessment and diagnostic skills
- Ability to provide supervision and guidance to other social work professionals
- Proficiency in conducting motivational interviewing sessions
