PROJECT VIDA is Hiring a Bilingual Integrated Health Care Navigator (English/Spanish) Near Chicago, IL

JOB SUMMARY

The Integrated Health Care Navigator (IHCN) is an experienced position with significant responsibilities, and a clear pathway for professional growth. The IHCN will manage Project Vida’s virtual PrEP care programming, conduct onsite HIV/STI testing and counseling and provide care navigation for those living with or at risk of HIV. The IHCN will ensure clients receive HIV/STI – related education services, sexual health information, insurance enrollment and/or medication assistance.

STARTING SALARY RANGE

$45,000 - $55,000 (commensurate with experience)

KEY RESPONSIBILITIES

Direct Client Services (35%)

  • Conduct walk-in HIV/STI counseling and testing for individuals and couples.
  • Conduct, Collaborate and facilitate client services between Project VIDA Inc. and other collaborators such as Howard Brown Health Center, The University of Chicago, Christian Community Health Center and Mt. Sinai Hospital.
  • Conduct individual level sessions providing risk/harm reduction counseling, health education and literacy.
  • Provide referrals and document outcomes of applicable program participants for HIV/STI/PrEP care and needed supportive services.
  • Provide linkage to care and health navigation to individuals testing positive for HIV and/or sexually transmitted infections.

Data Management (35%)

  • Track client sign ups, linkage to care, referrals and renewals for Project Vida’s virtual PrEP care programming (Mistr).
  • Submit required reports to supervisor and to funding sources within the scheduled time frame.
  • Ensure that the program goals, work plan, and scopes are met consistently and effectively.
  • Ensure the completeness and accuracy of all required program documentation and outcome measures

Outreach and Engagement (25%)

  • Conduct initial and follow up outreach to Project Vida’s virtual PrEP programming clients (Mistr).
  • Conducting digital outreach on social media, geo-dating sites and other digital platforms.
  • Support client retention for the agencies virtual PrEP care program.
  • Network with other community-based organizations, community members, health care facilities and city and state entities.

Additional Assignments (5%)

  • Attend required internal and external meetings, trainings, and conferences.
  • Complete continuing education on topic including HIV, STI, Hep A/B/C, Phlebotomy, etc.
  • Ensure that services are conducted in conjunction with funding source standards and requirements.
  • Adhere to agency, program and funding source policies and regulations.
  • Assist in generating additional resources and funds for Project VIDA programs including fundraising.
  • Perform other related duties as assigned by Supervisor

PRIORITY QUALIFICATIONS

  • AA with over two (2) years of relevant experience required. BA/BS Degree with at least one (1) year of experience in social services field preferred.
  • Bilingual in English and Spanish (Required)
  • Certified Phlebotomist (from a nationally accredited program)
  • Must be open to working days, evenings and Saturdays.
  • Previous experience in health education, public health, HIV prevention, or related field (Required).
  • Certified in HIV Counseling, Testing, and Referral Services (Required).
  • Culturally competent with LGBT, African American, and Latino populations (Required).
  • High proficiency in office Suite (i.e. Word and Excel) (Required).
  • Effective written and oral communication skills (Required).
  • Organizational and time management skills (Required).
  • Reliable transportation (Required).

Job Type: Full-time

Pay: $45,000.00 - $55,000.00 per year
