PROJECT SELF SUFFICIENCY is Hiring a Community specialist Near Newton, NJ

Experienced professional needed to develop and maintain relationships with agencies that serve pregnant people, children birth to five, and their families for the purposes of establishing reciprocal referral networks and generating representation on the Community Advisory Board.

Responsibilities :

  • all required meetings and training deemed necessary by DCF.
  • key contacts with local partner agencies and establish and maintain relationships, partnerships, and coalitions with community resources and services.
  • ongoing relationships and conduct routine follow-up meetings with the prenatal and pediatric care providers.
  • nurse home visitors as they connect families with community resources.
  • and monitor the Post Visit Connection process to identify unmet community resource needs and / or referrals, customer satisfaction, and problem solve solutions for the community.
  • the program by communicating with referral sources, groups that interact with families, and community leaders.
  • in relevant local and / or regional committees and task forces to communicate and collaborate regarding community early childhood and perinatal needs.
  • in joint community advisory board to assist in coordinating services within the early childhood system of care.
  • referral process and alignment of resources.
  • in quality improvement and evaluation activities.

Qualifications :

  • degree or bachelor’s degree with additional related experience.
  • written and verbal communication skills
  • to communicate clearly and professionally with a range of community stakeholders.
  • level of familiarity with county health and social services.
  • of and experience in communication, marketing, public relations, public speaking, and grant writing.
  • skills and a willingness to take initiative and be proactive.
  • to work independently, as well as in teams.
  • to accept personal differences, establish trusting relationships, and work with culturally diverse populations.
  • with Microsoft Office software and social media platforms.
