Progressive Quality Care is Hiring a Director of Social Services Near Aurora, OH

Our 5-star CMS awarded facility is looking for dedicated people to join our team. The Avenue is a Post-Hospital Rehabilitation and Skilled Nursing Care Facility that offers a distinctive environment not commonly found in skilled nursing facilities. From our hotel-like environment to our all private suites, the Avenue is committed to providing an excellent level of physical, medical, emotional and spiritual support in an environment of comfort and respect. Our facility provides some of the area’s highest level of rehabilitation, nursing and medical support to individuals needing post-hospital care and services. We are looking for a committed individual with a reputation for being cheerful and friendly.

The role of Director of Social Services will consist of assisting in meeting the mental, psychological, and psychosocial needs of our residents. This is a growing, fast-paced facility, so prior experience in social services within a long-term care setting is a must.

Major Duties

· Develop and implement social service policies and procedures to meet residents needs in compliance with federal, state, and local regulations.

· Obtain pertinent social data about personal and family problems medically related to the residents illness.

· Lead care plan meetings

· Act as a discharge planning coordinator for the health care center and each resident

· Completion of Social Services part of MDS using Point Click Care

· Maintain a working resident council, family council, ethics committee and grievance committee

· Prepare the initial Social Service Assessment on each resident including past, present, social and functional history and plan of care for each resident

· Assist and provide with identified referrals as needed (i.e, dental, hearing, vision, counseling, psychiatrist, and any other diagnostic service)

· Make arrangements for obtaining adaptive equipment, clothing, and personal items
