We are a private practice in the Sacramento area looking for a NP/PA, English/Vietnamese speaking Preferred. Our practice offers primary health care services.

• Must have an active NP, PA License for CA.
• National Certification, experience in primary care/adult/geriatric

Under the general direction of the Lead Physician the NP/PA will be responsible for providing services to Adult and Geriatric patients. The NP/PA will perform basic physical exams, record these findings using evidence-based information, the NP/PA plans and implement the appropriate treatment for the patient.

Essential Functions/Responsibilities:

• Provides ambulatory cute, chronic, and preventative medical services to patients
• Evaluates and treats patients
• Establishes and monitors appropriate level of care
• Completes all medical record documentation prior to end of shift
• Works with MD to ensure operations are consistent with medical and professional standards of care
• Meet with MD or designate to discuss quality of patient care, review policies, procedures, outcomes reports, and records.
• Participates in quality management processes including encouragement of clinical support staff to assure proficiency in the delivery of clinical support activities.
• Support process improvement initiative to enhance Team based care
• Educate patients on wellness, prevention and early detection by providing materials and resources
• Document all services accurately and completes billing/charges in 48 hours of visit
• Help MD with patients HEDIS, Annual Wellness Exams requirements

Knowledge, Skills, Abilities:

• Demonstrates knowledge and compliance of HIPAA regulations
• Demonstrates critical thinking and problem-solving skills
• Must possess excellent computer skills and EMR experience
• Must be able to work in a fast-paced environment
• Able to muti-task, prioritize tasks, meet deadlines. Requires analytical skills and demonstrates attention to detail.
• Strong verbal skills with patients and staff. Must be well organize and professional
• Ability to remain tactful under pressure and present a professional demeanor and communication style
