Job Description:

As a Home Health Care Community Liaison, you will play a pivotal role in expanding our client base and fostering relationships within the community. Your primary responsibility will be to generate referrals for our home health care services, focusing on securing straight Medicare referrals. You will leverage your existing book of business and cultivate new relationships with healthcare professionals, community organizations, and other referral sources.

Responsibilities:

• Develop and maintain relationships with physicians, hospitals, skilled nursing facilities, assisted living facilities, and other healthcare providers to generate referrals.
• Identify and pursue opportunities for business development within the community.
• Educate referral sources on the benefits and services offered by Proactive home care group.
• Collaborate with the Agency Administrator and marketing team to develop and implement targeted marketing strategies.
• Track and report on referral activity and outcomes.

Requirements:

• Proven track record of success in generating referrals for home health care services.
• Strong communication and interpersonal skills.
• Self-motivated with the ability to work independently and as part of a team.
• Knowledge of Medicare regulations and reimbursement processes preferred.
• Valid driver's license and reliable transportation.
• Must have clear criminal background check
