Job Summary
Coordinate the care for oncology patients, their family and/or caregiver across the continuum of care while overcoming healthcare barriers. The objective is to coordinate and provide high quality patient care across all care delivery sites. This may include coordination, facilitating, monitoring, and evaluating interventions to achieve desired outcomes.Accountabilities
Minimum Education
Bachelor's Degree - Nursing
Minimum Experience
3 years -\u00A0Oncology
Required Certifications/Registrations/Licenses
Licensed to practice as a Registered Nurse in South Carolina
One of the following oncology certifications:
OCN (Oncology Certified Nurse) from ONCC (Oncology Nursing Certification Corp); or
ONN-CG (Oncology Nurse Navigator-Certified Generalist) from AONN (Academy of Oncology Nurse & Patient Navigators); or
Bone Marrow Transplant Certified Nurse (BMTCN); or
Certified Breast Care Nurse (CBCN)
