Job Summary
This position reports to the Home Care Supervisor PACE and provides direct patient care in the community and at the PACE Center.Accountabilities
- Provides personal care to include, bathing, dressing, feeding, toileting, transfers, lifting and incontinent care, to the PACE participants in their homes.\u00A0- 60%
\u200B
- Provides care to participants at the PACE Center. - 20%
\u200B
- Responsible to report any changes in medical or physical status to the Home Care Supervisor PACE or designee. - 10%
\u200B
- Performs procedures/treatments\u00A0under supervision of HCS or RN. - 10%
Supervisory/Management Responsibilities
This is a non-management job that will report to a supervisor, manager, director or executive.
Minimum Requirements
- High School Diploma or equivalent
- 1 year - Nurse tech hospital experience or skilled nursing facility. Must have 1 year experience working with the frail or elderly.
Required Certifications, Registrations, Licenses
- SC CNA
- CPR
- Valid driver\u2019s license, an acceptable three-year motor vehicle record as defined by the Acceptable Motor Vehicle Record (MVR) Chart, and a certificate of auto insurance.
Required Knowledge, Skills, Abilities
- Basic computer skills
- Knowledge of office equipment (fax/copier)
