POSITION PURPOSEUnder the supervision of the Director of Clinical Services the On-Call/Triage Registered Nurse’s (RN) primary focus is on promoting and maintaining client’s health and well-being, preventing illness and injury, and managing acute and chronic health conditions within the client’s home or via the phone. The On-Call/Triage RN will complete tasks outside of regular office hours, including evenings, weekends, and holidaysORGANIZATIONThis position reports to the Director of Clinical Services.ESSENTIAL FUNCTIONS
  • Assessing patients' health status and developing individualized care plans for patients in their homes
  • Administering medications and treatments as prescribed by physicians and other healthcare providers.
  • Monitoring patients' vital signs and responding to changes in their condition.
  • Educating patients and their families about health conditions, medications, and self-care techniques specific to their home environment
  • Collaborating with other healthcare providers, such as physicians, nurse practitioners, and social workers, to coordinate patient care.
  • Documenting all patient interactions and interventions in the electronic medical record
  • Providing emotional support to patients and their families while respecting their cultural and religious beliefs
  • Participating in ongoing education and training to maintain knowledge of best practices in home health nursing care.
  • Facilitates appropriate upward and downward communication.
  • Communicates identified needs and potential solutions to appropriate administrator.
  • Promotes staff development and professionalism.
  • Supervises patient and family care as specified by the plan of care, assessing appropriateness, continuity, service, and quality of care.
  • Oversees to ensure that patient’s re-certifications are discussed and completed in a timely manner.
  • Promotes positive working relationship with family, physician, and other facilities.
  • Oversees the communication with the attending physician to ensure continuity and quality of care.
  • Participates in on-call duty as needed or as back up to on-call nurse, performs patient admissions and visits as needed.
  • Managing medication schedules and ensuring communication with the medical team to avoid medication errors.
  • Encouraging all staff to adhere to the highest standards in patient care and conducting regular staff reviews.
  • Maintaining knowledge of current trends in nursing and ambulatory care and educating teams.
  • Assessing, processing, and discharging patients.
  • Managing patient and employee records with a focus on following HIPAA regulations and quality standards
  • Answering phone calls from clients and caregivers and assessing their needs in a triage role.
  • Determining the appropriate level of care needed based on client’s symptoms and medical history.
  • Other duties as assigned.
KNOWLEDGE & EXPERIENCE
  • Graduate of accredited registered nurse program.
  • Current state licensure in Oklahoma and in good standing per state requirements.
  • Current Basic Cardiac Life Support (BCLS) required
  • Current Advanced Cardiac Life Support (ACLS) preferred
  • Certified Rehabilitation Registered Nurse (CRRN) preferred
  • 1-2 years recent Home Health nursing experience
  • Active Oklahoma driver’s license
  • Current car insurance and registration
  • Required vehicle

PHYSICAL REQUIREMENTSThe physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
