**Responsibilities:**

- Conduct patient assessments, diagnosis, and treatment in rehabilitation centers, primarily in private practices and some Medicaid offices.

- Collaborate with healthcare professionals to ensure patient care.

- Conduct thorough chart reviews before scheduled clinic days, aiding in the creation of concise patient summaries

- Utilize your cardiovascular expertise to enhance patient outcomes.

- Travel to various locations within North and South Jersey.

- Work closely with a dedicated tech companion.

**Requirements:**

- Nurse Practitioner certification with a focus on cardiovascular care.

- Own reliable transportation for daily travel.

- Comfortable with a personable and patient-centric approach.

- Openness to travel within the designated regions.
