Prime Home Health and Pacific Hospice & Palliative Care is Hiring an Intake Coordinator - Prime Home Health, FT Near San Antonio, TX

As a Home Health Intake Coordinator for Prime Home Health, you will play a crucial role in the efficient operation of a fast growing home health agency by coordinating the intake process for new patients. You will be responsible for gathering and verifying patient information, determining eligibility for home health services and ensuring a smooth transition from referral to admission. Your attention to detail, strong communication skills, and ability to work collaboratively within the team will contribute to our overall success. 

Responsibilities:

  1. Intake Process Management:
    • Receive referrals from healthcare professionals, hospitals, and other sources, and initiate the intake process.
    • Collect and review patient information, including medical history, diagnosis, medications, and insurance coverage, ensuring accuracy and completeness.
    • Verify insurance benefits and coverage, and coordinate with the billing department to ensure proper billing procedures are followed.
    • Communicate with referring physicians and healthcare providers to gather additional information as needed.
  2. Eligibility Determination:
    • Assess patient eligibility for home health services based on established criteria and guidelines.
    • Coordinate with clinical staff to evaluate the patient's medical condition and needs to determine if they meet the requirements for home health care.
    • Collaborate with insurance companies and payers to obtain authorization for services.
  3. Communication and Coordination:
    • Serve as the primary point of contact for patients, families, and referral sources throughout the intake process.
    • Liaise with healthcare professionals, case managers, and social workers to facilitate a smooth transition from referral to admission.
    • Collaborate with internal departments, including nursing, therapy, and scheduling, to ensure appropriate staffing and resources are available for new admissions.
    • Communicate effectively with patients and their families, providing clear and concise information about the home health services and answering any questions or concerns.
  4. Documentation and Record-Keeping:
    • Maintain accurate and up-to-date electronic records of all patient intake activities, ensuring compliance with regulatory requirements and agency policies.
    • Complete required forms, contracts, and consent documents, ensuring proper documentation for each patient.
    • Prepare and distribute reports, as necessary, to track and monitor the intake process and identify areas for improvement.
  5. Quality Assurance:
    • Participate in quality assurance activities, including audits and reviews, to ensure compliance with regulatory standards and agency policies.
    • Identify opportunities for process improvement and contribute to the development and implementation of best practices.

Requirements:

  • High school diploma or equivalent; additional education or certification in healthcare administration or a related field is a plus.
  • Intake experience in Adult Home Health preferred, or other healthcare setting or a similar field.
  • Proficient with Kinnser/Wellsky EHR system preferred. Use of other programs such MS Office and Slack are a plus. 
  • Strong knowledge of medical terminology, insurance verification, and eligibility requirements for adult home health services.
  • Excellent communication skills, both written and verbal, with the ability to effectively interact with patients, families, healthcare professionals, and internal staff.
  • Detail-oriented and organized, with the ability to manage multiple tasks and prioritize workload.
  • Knowledge of regulatory guidelines and compliance requirements related to home health services.
  • Demonstrated ability to work independently and as part of a team, with a customer-centric approach.
