Prime Home Health and Pacific Hospice & Palliative Care is Hiring a CNA Certified Nurses Aide - Adult Home Health PRN Near San Antonio, TX

As a Certified Nursing Assistant (CNA) at Prime Home Health, you will provide essential personal care and assistance to patients in their homes. You will play a vital role in supporting patients' independence, ensuring their comfort, and promoting their overall well-being in their San Antonio homes. Your compassionate nature, empathy, and commitment to delivering high-quality care will contribute to the positive patient experience and improved quality of life.

Responsibilities:

  1. Personal Care Assistance:
    • Assist patients with activities of daily living (ADLs) such as bathing, grooming, dressing, and toileting.
    • Provide assistance with mobility, transfers, and ambulation, ensuring patient safety and proper body mechanics.
    • Help patients with positioning, turning, and repositioning to prevent pressure ulcers and promote comfort.
    • Assist with range of motion exercises and other therapeutic activities as directed by the healthcare team.
  2. Medication Reminders and Assistance:
    • Remind patients to take their medications as prescribed by their healthcare providers.
    • Assist patients with medication administration, following established protocols and guidelines.
    • Document medication administration accurately and report any changes or concerns to the appropriate healthcare professional.
  3. Vital Signs Monitoring:
    • Measure and record patients' vital signs, including temperature, pulse, respiration, and blood pressure.
    • Report any abnormal findings or significant changes in vital signs to the supervising nurse or healthcare professional.
  4. Meal Preparation and Nutrition Support:
    • Assist patients with meal planning and preparation, considering their dietary restrictions and preferences.
    • Provide encouragement and assistance to ensure patients maintain adequate nutrition and hydration.
    • Follow specific diets or feeding plans as directed by the healthcare team.
  5. Light Housekeeping and Household Tasks:
    • Perform light housekeeping tasks such as cleaning, laundry, and changing linens.
    • Maintain a safe and clean environment for patients, including organizing and decluttering living areas.
    • Assist with grocery shopping and running errands for patients as needed.
  6. Emotional and Social Support:
    • Provide companionship and emotional support to patients, engaging in conversation and activities to promote mental well-being.
    • Foster a supportive and empathetic environment for patients, listening to their concerns and offering comfort.
  7. Documentation and Reporting:
    • Maintain accurate and timely documentation of patient care activities in Kinnser EHR system, including ADL assistance, vital signs, and observations.
    • Report any changes in patients' condition, safety concerns, or significant events to the supervising nurse or healthcare professional.

Requirements:

  • High school diploma or equivalent; 
  • Valid certification as a Certified Nursing Assistant (CNA) in the State of Texas. 
  • Knowledge of computers is mandatory, you will be using Slack for office communications, email and Kinnser EHR system for visits. You must have computer experience and be familiar with Kinnser or other EHR system. 
  • Current CPR certification
  • Experience as an Adult Home Health Aide is preferred.
  • Knowledge of personal care techniques and principles of infection control.
  • Excellent communication and interpersonal skills to interact effectively with patients, families, and healthcare professionals.
  • Ability to follow care plans, instructions, and safety protocols.
  • Reliable transportation to travel to patients' homes within the designated service area.
