Responsibilities
- Perform patient assessments and provide primary care services to individuals and families.
- Utilize electronic health record (EHR) systems to maintain accurate patient records and ensure continuity of care.
- Collaborate with healthcare team members to develop and implement patient care plans.
- Utilize communication skills to effectively interact with patients and their families.
- Stay updated on the latest medical advancements and best practices in the field.
- Provide health coaching and education to patients on various health topics.
- Ensure compliance with regulatory standards and ethical guidelines in all patient interactions.

Job Type: Full-time

Pay: $115,000.00 - $125,000.00 per year
