Primary Care of Central Florida is Hiring a FACILITY PATIENT ADVOCATE SPECIALIST Near Orlando, FL

We are a large practice with over 25 providers that serve Skilled Nursing Facilities, Assisted Living Facilities and our homebound patients in the comfort of their home in 14 counties. We are committed to providing excellent care to our patients and buildings while reducing hospital readmissions.

Our medical practice is currently looking for an outstanding Certified Healthcare Professional to join our Patient Advocate Team. The Patient Advocate Specialist will work closely with the Patient Advocate Manager and the Provider to develop a relationship with patients, their families, and caregivers. Compassionate communication with provider, color team, patients and caregivers, coupled with strong documentation of all activities, forms the core of this position. We assist as needed between provider visits. We are your go-to HPG Representative!

We are currently seeking a skilled and compassionate certified healthcare professional to join our field team. As a certified healthcare professional with us, you will play a vital role in providing high-quality care to our patients.

Responsibilities:

  • Assisting healthcare providers with patient care.
  • Assisting as needed between provider visits- finding a specialist if needed, assisting with urgent needs relaying requests to the provider and other non medical needs.
  • Thorough, accurate documentation of all activities.
  • Answering incoming calls and making outgoing calls.
  • Constant communication with nursing staff, providers, patients and their families/caregivers.
  • Utilizing Microsoft Excel to analyze trends and monitor patient census
  • Keeping up to date with healthcare policies to better serve patients
  • Ensuring a clean and organized clinical environment.
  • Adhering to all safety protocols and regulations.


Requirements:

  • Certification: CMA, RMA, CNA, MA or similar.
  • Previous experience in a medical setting preferred.
  • Strong communication and interpersonal skills.
  • Ability to multitask and work effectively in a fast-paced environment.
  • Excellent attention to detail and organizational abilities.
  • Commitment to providing compassionate care to patients.
  • Travel may be needed to multiple facilities.
