Job Title: Registered Nurse

Department: Adult Foster Care

Reports To: Director of Community Living

SUMMARY

The AFC provider must employ a licensed Registered Nurse to provide medical assistance for the AFC program, in compliance with 130 CMR 408.433(B)(2). Nursing services must be individualized to meet the needs of each member and must include all of the following activities. This position is an integral part of a multidisciplinary team.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following: (Other duties may be assigned)

1. Complete and sign the designated screening tool as required under 130 CMR 408.416© for MassHealth payment.

2. Complete the MDS-HC for intake, annually and upon significant change

3. Coordinate the implementation of physician’s orders with the member, AFC caregiver, and AFC provider personnel

4. Develop, sign and review on an ongoing basis each member’s AFC plan of care (within 30 days of admission);

5. Conduct on-site visits with each member at the qualified home setting:

a. for Level I, bimonthly (alternating with the bimonthly visit by the care manager);

b. for Level II, weekly upon admission and monthly, or more often as the member’s condition warrants;

6. Review the physician summary;

7. Monitor the health status and medication management of all members to ensure that all needed AFC is properly delivered;

8. Report changes in the health status of any member to the member’s physician;

9. Reviewing and sign AFC caregiver logs monthly;

10. Conduct on-site visits with each member at the qualified setting bimonthly (alternating with the bimonthly visit by the care manager) for Level I, and monthly or more often as the member’s condition warrants for Level II;

11. Complete a nursing progress note corresponding with each on-site visit or encounter, or more often as the member’s condition warrants;

12. Send a copy of the updated plan of care and a copy of the member’s semiannual health-status report to the member’s physician for review and approval.

13. Plan for and implementing discharges from the AFC program;

14. Conduct an orientation for each AFC caregiver before the AFC caregiver begins personal care;

15. Provide ongoing training to AFC caregivers on health, medications, safety and aging.

16. Educate the member about hygiene and health concerns;

17. Select, train, evaluate, and supervise AFC caregivers in conjunction with the care manager;

18. Review each member in its care to ensure that the clinical criteria for MassHealth payment for AFC continue to be met.

19. Assess, along with the multidisciplinary team, the member’s ability to manage safely in the qualified setting for a maximum of three hours a day without the presence of a qualified caregiver;

20. Report to Director any incidence or accident, change in social or clinical status of member, change in eligibility of member or caregiver, medications issues or errors, MassHealth required information, or other programmatic information

21. All other duties as related to the PRIDE AFC Program development.

QUALIFICATIONS To perform this job successfully, an individual must be able to perform each essential duties satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Reasonable accommodations are available as required.

EDUCATION and/or EXPERIENCE

The Registered Nurse must have at least two years’ recent experience in the direct care of the elderly or disabled adults.

LANGUAGE SKILLS

Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers and the general public.

MATHEMATICAL SKILLS

Ability to work with mathematical concepts. Ability to apply concepts such as banking recordkeeping, adding receipts, doing budgets, fractions, percentages, ratios, and proportions to practical situations.

REASONING ABILITY

Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

CERTIFICATES, LICENSES, REGISTRATIONS, PERSONNEL REQUIREMENTS

Valid Driver’s Licenses, recent physical exam within 12 months. TB test prior to start. The registered nurse must be fully licensed by the Massachusetts Board of Registration in Nursing.
