Job Title: Licensed Master Social Worker

Shift: Days

Pay Range: $55 to $60/Hour.

Job description:

· Provides social work services to patients and families to assist them in continuity of care planning and in enhancing their capabilities of coping with illness and daily functioning.

· Collaborates with Hospital, and community-based personnel to ensure efficient service delivery and a high level of customer satisfaction.

· Discharge planning experience needed.

Education:

· Master’s in social work

Licenses and Certifications:

· LMSW required, LCSW preferred.
