POSITION SUMMARY/RESPONSIBILITIES
Under the general direction of clinical director/area manager, functions independently in the role of expert community health clinician, educator, consultant, case manger, and/or researcher. As part of a multi-disciplinary team, develops a broad spectrum plan of care for individual/families/ population group. Provides specialized health care to clients in the defined population. Identifies emerging/recurring health problems. Develop and/or implements programs both internally and in collaboration with external partners for identified problems. Supervise nursing and non-professional staff.
EDUCATION/EXPERIENCE
Bachelor of Science in Nursing and two years of experience in nursing is preferred. Recent experience in community health, maternal/child health, or mental/behavioral nursing and proficiency in reading, writing and speaking Spanish preferred. Ability to interpret findings, initiate and follow established protocols and procedures, consult with members of healthcare team, observe and assess health factors.
LICENSURE/CERTIFICATION
Current license to practice professional nursing in the State of Texas is required. Valid Texas driver’s license and insured automobile required. Current CPR required
