Organizational Summary:

Prevention Point Philadelphia (PPP) is a multi-service public health organization dedicated to protecting the health and welfare of the homeless, drug users, sex workers, and the uninsured. Using a harm reduction approach, PPP offers culturally sensitive, non-judgmental services to address the health and social service needs of drug users and sex workers in Philadelphia. PPP meets people where they are, providing information and access to resources with dignity and respect. PPP promotes harm reduction through mobile medical care, sterile syringe exchange, referrals to social services, and comprehensive prevention case management services.

Program Overview:

The Integrated Health Care Management Program (IHCM) supports high-needs participants at PPP who face challenges accessing medical and behavioral health services for multiple diagnoses and who need specialty care coordination, escort, and advocacy with both internal and external health systems.

Through intensive case management, the program focuses on providing low-barrier services to those who have had difficulty engaging in care in traditional settings. IHCM sources participants through team outreach and other low-barrier services at PPP including SSP, Drop-In, and Homeless Outreach.

Position Overview:

The Complex Care Manager works with participants with co-occurring issues such as physical and/or behavioral health, substance use disorder, homelessness, and/or IDD, providing a trauma-informed and harm-reduction approach to participant-centered care. The Care Manager meets the individuals where they are, assessing individuals' medical, mental health, housing, and psychosocial needs, and creates and implements care plans that blend these needs with any additional participant-identified goals. The Care Manager assists with linking and accessing services and care, both internally and externally, monitoring and coordinating care delivery, crisis de-escalation and problem resolution, and utilizing community resources to support participant's growth.

Position Duties and Responsibilities:

• Conduct benefits assessment and enrollment, review medical and behavioral health histories, coordinate integrated health services and specialty care access, and provide mobile escort, advocacy, and in-person support at external physical and behavioral health resources.
• Conduct participant interviews to assess medical and psychosocial needs. Use these assessments to inform and create collaborative care plans and goals.
• Coordinate care with external providers, hospitals, social workers, and treatment centers for individuals both inpatient and outpatient, assisting with patient advocacy and care plans.
• Collaborate with other PPP departments such as STEP, TALC, PrEP, HIV, HCV, Housing, Drop-In, and PAD to ensure continuity of care for participants engaged in multiple PPP programs.
• Assist with crisis de-escalation and coordinate response to mental health crises.
• Visit with participants face-to-face in the community and around PPP.
• Meet program-specific documentation and data entry requirements, as discussed with immediate supervisor, by monthly deadlines.
• Facilitate referrals to both internal and external social services.
• Escort patients to off-site appointments as needed.
• Actively participate in supervision, team, and staff meetings.
• Work in a fast-paced environment and assist with additional operational needs including drop-in center coverage.
• Perform other duties as assigned by supervisor.

Qualifications: 

• Belief in promoting the health and dignity of individuals who use drugs through a harm-reduction lens.
• Bachelor’s degree in public health, social work, psychology, or a related field preferred.
• A minimum of 2 years of experience working in the field of case management, outreach, or other related public health field.
• Experience with working with individuals experiencing homelessness impacted by substance use, mental health, and/or complex medical issues.

Skills:

• Spanish language skills preferred; cultural competence required.
• Skilled in communicating effectively with a variety of people of different backgrounds.
• Strong computer and professional writing skills.
• Ability to use a harm reduction approach with people who use drugs and engage in sex work.
• Dynamic, creative, flexible, able to learn and adapt to a changing environment.
• Ability to work independently and as part of a team.
