Organizational Summary:
Prevention Point Philadelphia (PPP) is a multi-service public health organization dedicated to protecting the health and welfare of the homeless, drug users, sex workers, and the uninsured. Using a harm reduction approach, PPP offers culturally sensitive, non-judgmental services to address the health and social service needs of drug users and sex workers in Philadelphia. PPP meets people where they are, providing information and access to resources with dignity and respect. PPP promotes harm reduction through mobile medical care, sterile syringe exchange, referrals to social services, and comprehensive prevention case management services.
Program Overview:
The Integrated Health Care Management Program (IHCM) supports high-needs participants at PPP who face challenges accessing medical and behavioral health services for multiple diagnoses and who need specialty care coordination, escort, and advocacy with both internal and external health systems.
Through intensive case management, the program focuses on providing low-barrier services to those who have had difficulty engaging in care in traditional settings. IHCM sources participants through team outreach and other low-barrier services at PPP including SSP, Drop-In, and Homeless Outreach.
Position Overview:
The Complex Care Manager works with participants with co-occurring issues such as physical and/or behavioral health, substance use disorder, homelessness, and/or IDD, providing a trauma-informed and harm-reduction approach to participant-centered care. The Care Manager meets the individuals where they are, assessing individuals' medical, mental health, housing, and psychosocial needs, and creates and implements care plans that blend these needs with any additional participant-identified goals. The Care Manager assists with linking and accessing services and care, both internally and externally, monitoring and coordinating care delivery, crisis de-escalation and problem resolution, and utilizing community resources to support participant's growth.
