Pinnacle Rehabilitation & Health Center, located in Smyrna, DE, has an exciting career opportunity for an experienced Director of Social Services to provide services to the residents in our 151 bed facility!!
As the Social Services Director, your responsibilities will include:
- Working with residents to identify their psychosocial, mental, and emotional needs
- Work with residents and family members to find community resources
- Review and set policies concerning resident care and quality of life
- Discharge planning
- Complete Social Services MDS reporting
- Care planning and assesments
- Resident Advocate
- Interdisciplinary team meetings
- Manage other employees in the social services department
Education
- Must possess, as a minimum, a Bachelors Degree in social work.
Experience
- Must have, as a minimum, two (2) years experience in a supervisory capacity in a hospital, nursing care facility, or other related medical facility.
