*Ensures an adequate stock of medications, medical supplies, equipment, etc., is maintained on the unit at all times to adequately meet the needs of the resident and notifies appropriate personnel of needs.
*Delivers routine daily oral, inhalation and topical medications to residents under direct supervision of a licensed nurse unless otherwise allowed by state law. Observes for any adverse drug reactions and reports any problems to the nurse. Records medication administration according to company policies and procedures. Follows written or verbal instructions on how to manage medications.
*Observes residents for changes and documents changes in their condition accordingly. Performs tasks associated with medication administration, including vital signs, medication dose calculations, stocking of the medication carts, and removal of discontinued medications.
*Practices safe handling and storage of medications. Responds to resident requests and other direct care duties as assigned. Identifies, manages, and treats specific skin conditions, such as pressure injuries; diabetic, venous, or arterial ulcers; and traumatic or complicated wounds. Provide wound care on assigned residents, in accordance with physician orders, following manufacturer recommendations and appropriate techniques. Completes a thorough and accurate wound assessment upon notification of wound. Completes follow-up assessments weekly and as needed.
*Documents all assessments in the medical record. Confers with the care plan team in the development of the comprehensive care plan. Documents resident care problems, abnormal findings, and changes in resident’s condition.
*Report to physician, supervisor, and resident/representative. Monitors implementation of preventive measures such as turning/positioning, pressure relief devices, nutrition/hydration measures, and toileting/incontinence care.
*Provides education to resident/representative as needed on topics concerning nutrition, preventive measures, and wound healing.
