JOB SUMMARY

The Youth Mentor works with youth and their families providing mentoring services per the youth’s plan. Youth mentors are responsible for ensuring that youth they work with receive caring, quality, and appropriate mentoring and supportive services. The goal of mentoring is to assist the youth to improve their social/emotional functioning, manage their behavior and symptoms, improve their perceptions of parental relationships, and improve their self-esteem.

DUTIES AND RESPONSIBILITIES

• Engages andcommunicates effectively with youth and their families/legal guardians, and all team members.
• Provide mentoring support to youth identified by the referring agency and deemed eligible for program.
• Assists consumers in crisis situations by providing de-escalation and other skills learned in training. Utilizes consumers’ crisis safety plan.
• Provides support by assisting consumers in identifying and resolving problems and makes effective use of resources.
• Participates in the development/reviews of individualized plans for youth.
• Documents all activities per policy.
• Develops healthy, mentoring relationships with youth.
• Maintains regular contact with each youth participating in activities based on their individualized plan.
• Regularly reviews each youth's progress toward meeting goals outlined in their individualized plan.
• Role models pro-social behaviors for youth.
• Provides transportation to youth (and families as required) to and from appointments and activities.
• Attends meetings with youth and their families, e.g., school, medical, mental health, substance abuse, court, etc. as required.
• Advocates for youth within a variety of settings, including, but not limited to school, juvenile court, medical and mental health agencies, and other community-based service providers.
• Works closely with families of consumers to provide appropriate support and access to resources such as family support groups.
• Maintains a working knowledge of Center policies, procedures, and licensure regulations governing Prestera’s service delivery.
• Adheres to confidentiality, HIPAA, and risk management policies and procedures including but not limited to completing Incident Reports.
• Utilizes technology provided to facilitate services and/or perform job efficiently and effectively.
• Meets training requirements as established, including but not limited to, CPR-First Aid and crisis intervention.
• Participates in other functional work by participating in related projects, as applicable and directed by Center leadership.

SUPERVISORY RESPONSIBILITIES

No direct supervisory responsibilities are required.

Qualifications

QUALIFICATIONS AND REQUIREMENTS

EXPERIENCE, KNOWLEDGE, AND SKILL REQUIREMENTS:

• Minimum of one year work with at risk youth preferred.
• Valid driver’s license required.
• Commitment to supporting and working with youth and families.
• Experience or prior training working with mental health and substance abuse population is preferred.
• Knowledge of community resources preferred.
• Ability to learn, navigate, and work daily in the EHR system effectively.
• Excellent interpersonal skills and ability to develop trusting relationships and handle sensitive and confidential situations.
• Possess good written and verbal communication skills in conjunction with good time management and organizational skills.
• Has shown ability to be creative and think outside the box in situations.
• Must be able to lift to 25 lbs.

EDUCATION AND TRAINING REQUIREMENTS:

• High School Diploma or General Educational Development Certificate required.
• Maintains First Aid/CPR certification.
• Maintains current crisis prevention/intervention training.
