Summary of Functions: Compiling lists of individuals or groups requesting space for activities, and schedules needed facilities / equipment. Completes IDD packets. Required to provide QMRP services.
Essential Duties and Responsibilities.
1. Completes IDD packets for potential individuals.
2. Coordinate and implement services for ID consumers
3. Make individual program plan and provide linkage to internal/external services.
4. Provide QMRP services including behavior Support services, psychological updates, social history updates etc.
5. Review Residential programs, day habilitation and supported employment
6. Program planning for Residential programs, day habilitation and supported employment.
7. Other duties as assigned.
Supervisory responsibilities:
This job has no supervisory responsibilities.
Education and/or experience. Bachelor's degree (B. A.) in a human services field from four-year college or university. Or one to two years related experience and/or training; or equivalent combination of education and experience.
Mathematical skills. Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Compute ration, percentages etc.
Language Skills: Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization.
Reasoning ability. Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form
Certificates, licenses, registrations. Must attain and maintain CPI and CPR certification.
Must be eligible for social work license.
