Beautifully renovated Nursing Home is seeking a compassionate and experienced Social Services Director to join our team. As the Social Services Director, you will play a vital role in providing essential support and assistance to our residents and their families.
Responsibilities of the Social Services Director:
- Conduct comprehensive assessments of residents' psychosocial needs and develop personalized care plans in collaboration with the interdisciplinary team.
- Provide emotional support, counseling, and resources to residents and their families to address their social and emotional well-being.
- Assist residents in accessing community resources and services to meet their individual needs and preferences.
- Facilitate family meetings and serve as a liaison between residents, families, and the facility to ensure open communication and understanding.
- Collaborate with medical professionals, including doctors, therapists, and social workers, to ensure the holistic care of each resident.
- Assist residents with discharge planning and coordinate the transition to home or other care settings when necessary.

Qualifications of the Social Services Director:
