The Village at Summerville is seeking a dedicated Director of Social Services to join our team! The ideal candidate will play a crucial role in assisting residents and their families, ensuring seamless transitions within our community. Responsibilities include, but not limited to, the following:
Uphold the PCSC Mission, Vision, and Values Statement in all aspects of work, leading by example and holding self-accountable for exemplifying compassionate Christian service.
Facilitate trust, initiating assessment of resident and family psychosocial status and coordinating and clarifying patient-specific financial aspects of care.
Implement and conduct resident orientation for Independent Living, Assisted Living and Memory Care.
Assist residents and their families with future planning and provide education on the aging process.
Assist residents with their adjustment to their new living conditions.
Serve as a first point of contact related to concerns of residents and/or family members.
Encourage family contact and communicate with families on an as needed basis; assist residents with various forms of communication to family.
Requirements Minimum Education, Experience and Skills: •Bachelor's Degree in Social Work •1 year of social work experience in long term care setting •Intermediate computer skills •Valid SC Drivers License Preferred Education, Experience and Skills: •Master's Degree in Social Work •Licensed as a Social Worker in South Carolina •3 years social work experience in a long term care setting At the Village, we believe in Relationships, Teamwork, Service, Excellence and Stewardship and use these values daily as we serve.
