The Village at Summerville is seeking a dedicated Director of Social Services to join our team! The ideal candidate will play a crucial role in assisting residents and their families, ensuring seamless transitions within our community.
Responsibilities include, but not limited to, the following:

• Uphold the PCSC Mission, Vision, and Values Statement in all aspects of work, leading by example and holding self-accountable for exemplifying compassionate Christian service.
• Facilitate trust, initiating assessment of resident and family psychosocial status and coordinating and clarifying patient-specific financial aspects of care.
• Implement and conduct resident orientation for Independent Living, Assisted Living and Memory Care.
• Assist residents and their families with future planning and provide education on the aging process.
• Assist residents with their adjustment to their new living conditions.
• Serve as a first point of contact related to concerns of residents and/or family members.
• Encourage family contact and communicate with families on an as needed basis; assist residents with various forms of communication to family.

Requirements
Minimum Education, Experience and Skills:
•Bachelor's Degree in Social Work
•1 year of social work experience in long term care setting
•Intermediate computer skills
•Valid SC Drivers License
Preferred Education, Experience and Skills:
•Master's Degree in Social Work
•Licensed as a Social Worker in South Carolina
•3 years social work experience in a long term care setting
