Premier Behavioral Health Center is Hiring a Behavioral Health Care Manager (LCSW, LMFT, MSW) Near Orange, CA

Social Worker to provide behavioral health care management services for a senior population both inpatient and at patient’s home. Responsible for the assessment and management of seniors with psychosocial, mental, and behavioral health needs. Conducts comprehensive assessments and on-going monitoring to ensure industry-best standards of care coordination.

  • Develops and initiates patient-centered care plan that is comprehensive and consistent with Medicare and health plan guidelines and policies and procedures.
  • Conducts home visits, hospital & skilled nursing facility visits.
  • Identifies, arranges for, and monitors appropriate services based on a good knowledge of Medicare programs.
  • Establishes and maintains a care management relationship with patients and their support network as appropriate.
  • Provides crisis intervention, advocacy, problem solving and therapeutic interventions.
  • Meets with patients at least monthly, and more often as clinically indicated. Reviews and modifies their care plan on an ongoing basis.
  • Documents progress and notes all case management activity regarding identified problems in a timely manner.
  • Maintains required paperwork and follows a clear, concise, and consistent system of charting to allow for continuity of care.
  • Educates patient and their support network about available community resources.
  • Establishes and maintains open and effective communication with network providers, including physicians, other health care specialists, and social workers.
  • Provides appropriate information on all significant aspects of individual member care and program operations, while maintaining necessary confidentiality.
  • Monitors the quantity and quality of the services provided by other involved providers.
  • Works closely with the behavioral health case management team, continuously evaluates the member’s ability to remain safely at home; coordinates placement as appropriate, according to Medicare and health plan guidelines.
  • Attends and actively participates in behavioral health team meetings, activities and problem-solving endeavors; contributes to open lines of communication within the behavioral health team.
  • Utilizes supervision appropriately; maintaining open lines of communication and providing updates on caseload activity.

Experience:

  • One year working with individuals with mental and/or behavioral health diagnoses and substance abuse disorders required.
  • Experience with and understanding of the medical and psychosocial problems of functionally/behaviorally impaired adults and older adults.
  • Exceptional communication and presentation skills relating to behaviorally/mentally impaired adults and older adults, their support systems, and other health care professionals.
  • Demonstrates case management skills and experience in the community health care delivery system.
  • Detail oriented with good problem-solving skills and the ability to prioritize multiple tasks.

The pay range for this position at commencement of employment is expected to be between $52,000 - $85,000 per year; however, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, skills, and experience. The total compensation package for this position may also include other elements, including a sign-on bonus, and discretionary awards in addition to a full range of medical, financial, and/or other benefits (including 401(k) eligibility and various paid time off benefits, such as vacation, sick time, and parental leave), dependent on the position offered. Details of participation in these benefit plans will be provided if an employee receives an offer of employment.
