Job Description:
The Registered Nurse (RN) in a non-medical home care agency plays a crucial role in ensuring the well-being and safety of clients within their homes. They are responsible for coordinating and providing high-quality care services to clients while adhering to established standards and regulations. The RN collaborates with other healthcare professionals, caregivers, and families to develop and implement care plans tailored to each client's needs.
Duties:
Client Assessment: Conduct comprehensive assessments of clients' health status, including physical, mental, and emotional well-being, to identify their care needs and develop personalized care plans.
Care Planning: Collaborate with clients, their families, and other healthcare team members to develop individualized care plans based on the assessment findings and the client's preferences.
Care Coordination: Coordinate and oversee the delivery of care services by home health aides, caregivers, and other healthcare professionals to ensure continuity of care and adherence to care plans.
Medication Management: Provide medication management services, including medication administration, monitoring for adverse reactions, and educating clients and caregivers on medication regimens.
Health Monitoring: Monitor clients' vital signs, symptoms, and overall health status regularly to identify changes or potential issues and intervene as needed.
Education and Training: Educate clients and their families on disease management, self-care techniques, safety precautions, and other health-related topics to promote optimal health and well-being.
Documentation: Maintain accurate and up-to-date documentation of clients' health assessments, care plans, interventions, and progress notes in compliance with regulatory standards and agency policies.
Communication: Communicate effectively with clients, families, caregivers, and other healthcare team members to ensure the delivery of coordinated and client-centered care.
Crisis Management: Respond promptly and effectively to medical emergencies, changes in clients' conditions, and other critical situations to ensure clients' safety and well-being.
Quality Improvement: Participate in quality improvement initiatives, performance evaluations, and ongoing professional development activities to enhance the quality and effectiveness of care services.
Experience:
Bachelor of Science in Nursing (BSN) or Associate Degree in Nursing (ADN) from an accredited nursing program.
Current and unrestricted Registered Nurse (RN) license in the state of practice.
Previous experience in home health, community health, or related healthcare settings preferred.
Strong clinical assessment and critical thinking skills.
Proficiency in developing and implementing individualized care plans.
Excellent communication, interpersonal, and teamwork skills.
Ability to work independently and collaboratively in a dynamic healthcare environment.
Knowledge of regulatory requirements and best practices in home care services.
CPR certification and other relevant certifications may be required.
