Precious Home Healthcare LLC is seeking full-time and part-time Registered Nurses to join our team in providing exemplary skilled nursing care to patients on an intermittent basis in their homes. All care is provided in accordance with physician orders and plan of care under the direct supervision of the Director of Clinical Services/Nursing Supervisor. Work hours may vary based on the needs of the assigned patients, and nights, weekends, and holidays may be worked as assigned.

The role of the RN is to ensure that all services are provided in accordance with the plan of care. Initial evaluations are made upon the first visit, and reevaluations are made regularly thereafter, with revisions being made to the plan of care as needed. In addition to providing total patient care, RNs are responsible for:

• Preparing clinical and progress notes for each patient visit and summaries of care conferences in a timely manner
• Counseling the patient and family in meeting nursing and related needs.
• Participating in and presenting in-service programs.
• Processing orders and notifying physician of patient needs and changes in condition.
• Completing certification/recertification orders and discharge summaries.
• Making referrals to Physical Therapist, Speech Language Pathologist, Occupational Therapist and Medical Social Worker if needed
• Supervising and training other nursing personnel (LPNs and Home Health Aides)
• Conducting patient care conferences
• Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant change in condition in accordance with Agency defined time frames.

To be qualified for this position, you must be a graduate of an accredited school of professional nursing and currently be licensed in the state of Ohio. Two years of nursing experience is preferred. Because you will be providing healthcare to patients in their homes, reliable transportation is required. Because some patients may need support turning or being repositioned, this position does require a level of physical activity including heavy lifting, extensive bending, and standing.

All applicants must be able to pass a BCI background check and two-step PPD Tuberculosis test at the applicant's expense. Proof of CPR certification is required.
