We are a home health & hospice agency providing services in clients' homes from Green Bay to Burlington and from Waukesha to Lake Michigan.
Our home office is located in Germantown, with branch offices in Fond du Lac, Appleton, and Waukesha.

We are currently in need of a Full-Time Speech Therapist for our
Kenosha and Racine County territories.

SIGN ON BONUS UP TO $5,000 PAID IN INSTALLMENTS

Our clients reside in assisted living facilities, their private home, or wherever home may be. We bring the care to them.
Job responsibilities include, but are not limited to, the following:

• Admitting new clients to our services

• Resuming client care after a recent hospitalization or recertifying client care at the end of episode

• Ability to utilize many different and unique therapy interventions and work closely with families in their home setting

• Communicating successfully, effectively, and efficiently with clients, families, doctors, facility staff, and co workers is a must to be successful in this role

• Ability to work independently but under the supervision of regional therapy leaders

• Good organization and time management skills are also critical

• Timely documentation is a must

• Completion of Oasis document

As a home health speech language therapist, you will be spending time in your car, and you will have a level of autonomy in scheduling your visits. Charting is done electronically. Our goal is for the majority of all charting to be done at bedside to allow you a work/life balance. HomeCare HomeBase is our electronic medical records system.
