Physician Practice Enhancement is seeking an experienced Primary Care or Geriatric Physician for a Medical Director position at a PACE program in Oceanport, NJ

PACE stands for Program of All-inclusive Care for the Elderly. It is an innovative Medicare program that provides individuals age 55 and older comprehensive medical and social services coordinated and provided by an interdisciplinary team of professionals in a community-based center and in their homes, helping program participants delay or avoid long-term nursing home care. Each PACE participant receives customized care that is planned and delivered by a coordinated, interdisciplinary team of professionals working at the center. The team meets regularly with each participant and his or her representative in order to assess the participant's needs. A participant's care plan usually integrates some home care services from the team with several visits each week to the PACE center, which serves as the hub for medical care, rehabilitation, social activities and dining.

We are seeking a skilled and compassionate Geriatric or Primary Care Physician/Medical Director to join our healthcare team. As a Geriatric/Primary Care Physician, you will provide specialized medical care to elderly patients, focusing on their unique healthcare needs and promoting their overall well-being. Responsibilities:
- Conduct comprehensive medical assessments and evaluations of geriatric patients
- Develop personalized treatment plans based on individual patient needs
- Monitor and manage chronic conditions commonly found in the elderly population
- Coordinate with interdisciplinary healthcare team members to ensure holistic care
- Provide preventive care services, including vaccinations and health screenings
- Educate patients and their families on disease management, lifestyle modifications, and healthy aging practices
- Collaborate with specialists as needed for specialized care or consultations
- Stay updated on the latest advancements in geriatric medicine through continuous learning and professional development

Monday - Friday position - no weekends - no holidays

Must have a valid NJ Medical license

Experience in Geriatrics a plus
