Collaborate with physicians, hospitals, and care facilities to provide coordinated and quality home-based services. Be the beacon of support and guidance in our healthcare landscape. This position will support the Northern Houston area (Conroe, The Woodlands, Spring, Cypress, North Humble).

Assist discharge coordinators in coordinating home care services, empowering patients to make informed choices for their wellbeing.

Provide patients and families with compassionate education on home care regulations and services.
