JOB SUMMARY:

The Prevention Specialist- PrEP Coordinator is responsible for the planning and implementation of all prevention services related to PrEP including, but not limited to, education, assessment, evaluation, enrollment, data collection, financial assistance, and follow-up, and will work closely with the medical team for laboratory evaluation and prescription fills.

Requirements

ESSENTIAL FUNCTIONS:

Duties and Responsibilities

  • Screen calls from persons interested in getting more information or starting PrEP, and meet with clients during their HIV testing - visit to evaluate for PrEP enrollment. Schedule an enrollment meeting with the client to complete enrollment paperwork, and to evaluate for the appropriateness of enrolling in PrEP services.
  • Conducts HIV Counseling (CTL) sessions in a non-judgmental manner and with fidelity to the core elements and core components of the HIV Prevention Counseling standards.
  • Assist with follow-up to ensure that persons diagnosed as living with HIV are successful linked to care and partner services.
  • Provide effective condom education. Distribute condoms to target HIV-infected persons and persons at highest risk of acquiring HIV. Provides education on pre-exposure prophylaxis (PrEP) and ensures persons eligible for PrEP if interested are linked to PrEP services.
  • Conduct a financial assessment to identify available financial resources for the client, and identify what additional resources the client may need.
  • Assist clients with completing any financial forms for PrEP Medication access.
  • Remain current on Gilead’s co-pay assistance program and the Advancing Access program, and assist clients with applying to the correct program for assistance.
  • Remain up to date on PAF and PAN financial assistance requirements, and help client’s complete forms and submit documentation for assistance.
  • Track client caseload and update paperwork and documentation from client as needed to ensure no disruption in PrEP access.
  • Assist with patient Retention by following up missed appointments and missed pharmacy refills.
  • Be able to answer general questions about PrEP for both agency staff and for clients, attend community events to promote PrEP, and assist with PrEP marketing strategies.
  • Maintain PrEP statistics and data about clients and program, and be able to report those data back to management as needed.
  • Network with other agencies, funders, and others to promote Positive Impact Health Centers’ HIV Prevention services.
  • Participate in monthly agency staff meetings, monthly departmental meetings and occasional health education trainings.
  • Leads and/or supports the organization of special events as assigned.
  • Perform other duties as assigned.

Knowledge, Skills, and Abilities

  • Must possess excellent interpersonal skills and the ability to communicate effectively in a multi-cultural setting including diverse clients, volunteers, agency staff and Board of Directors.
  • Successful candidate will be a self-starter with the ability to multi-task.
  • Possess excellent communication skills (writing, spelling, listening and speaking).
  • Cooperates as a team member.
  • Possesses problem solving and decision-making abilities.
  • Ability to work without close supervision.
  • Utilizes good management techniques.
  • Proficiency in word processing, spreadsheet database and presentation software.
  • Abides by all policies and procedures concerning personnel including conflict of interest policy.
  • Knowledge of HIV/AIDS, knowledge of the target population and past direct experience with people living with HIV/AIDS a plus.
  • This position will require working some evenings and weekends based on weekly schedule as provided by program manager.

Minimum Qualifications

MINIMUM Associate’s degree from an accredited institution; Bachelor's degree preferred, in a public health, counseling, psychology, sociology or related field

Experience

Two years of experience working in the field of HIV or public health or any

equivalent combination of proven training and experience which provides the required knowledge, skills, and abilities to perform this job
