PORCHLIGHT is a 501(c)3 nonprofit organization founded in 1993 to provide a warm, safe, and hospitable place for single men to sleep and be nourished with three healthy meals each day. Today, with phenomenal partnerships and support from King County’s Eastside community, PORCHLIGHT serves more than 1,700 people each year with street outreach, day center services, shelters, permanent subsidized housing, case management, and other life-saving supportive services—including 125,000 meals. PORCHLIGHT is a healthy, fiscally stable, organization with a dedicated Board and staff poised to advance its mission.

This is a very exciting time for PORCHLIGHT as the organization engages the wider community in new and impactful ways to foster hope, dignity and the building blocks that provide stability to people in need on our streets. For decades, we have quietly worked in our community, providing outreach, shelter, permanent housing, services & community.

OVERVIEW OF POSITION:

The Nurse Practitioner (NP) plays a crucial role in providing comprehensive healthcare services to patients across the lifespan dedicated to PorchLight’s mission and values. Working independently as a practitioner and collaborating with the behavioral healthcare team, the NP delivers high-quality medical care, emphasizing preventive measures, health promotion, and patient education to our diverse patient population. With manager approval, the NP can flex time to perform responsibilities via Telehealth.

KEY RESPONSIBILITIES

Clinical Practice:

  • Conduct comprehensive health assessments, including physical examinations, detailed medical history reviews, and utilization of appropriate diagnostic tests.
  • Utilize critical thinking skills to diagnose acute and chronic health conditions across diverse populations, encompassing diabetes, hypertension, infections, and injuries.
  • Develop and implement individualized patient-centered treatment plans based on evidence-based guidelines and best practices.
  • Prescribe medications, order and interpret laboratory and diagnostic tests, ensuring accurate and timely analysis.
  • Continuously monitor patient progress, adjusting treatment plans as necessary to optimize health outcomes.

Patient Education and Collaboration:

  • Empower patients through comprehensive education on disease prevention, healthy lifestyle choices, and self-management techniques.
  • Collaborate with patients to establish personalized health goals, fostering shared decision-making and accountability.
  • Work collaboratively with physicians, nurses, social workers, and other healthcare professionals to ensure continuity of care and a coordinated approach to patient well-being.
  • Actively participate in case conferences and interdisciplinary team meetings to contribute clinical expertise and insights.

Advocacy and Community Engagement:

  • Champion patient-centered care, advocating for their physical, emotional, and spiritual well-being.
  • Facilitate effective communication between patients, families, and the entire healthcare team to ensure clear understanding and informed decisions.
  • Engage in community health initiatives, participate in health fairs, and contribute to educational programs to promote preventative care and population health.
  • Champion health equity by promoting access to quality healthcare and addressing social determinants of health within the community.

KNOWLEDGE, SKILLS, AND ABILITIES

  • Master's degree in nursing with a Nurse Practitioner specialty.
  • Current Washington State Nurse Practitioner license with no restrictions or pending disciplinary actions.
  • Minimum of one year of experience as a Nurse Practitioner in a clinical setting preferred.
  • Strong clinical skills in history taking, physical examination, diagnosis, and treatment planning.
  • Excellent communication, collaboration, and interpersonal skills.
  • Commitment to providing patient-centered care and building rapport with diverse populations.
  • Proficient in electronic health records (EHR) systems.

CERTIFICATES, LICENSES & REGISTRATIONS

  • National certification as a Nurse Practitioner in a relevant area, as recognized by the Washington State Nursing Commission.
  • Current, active Washington State Nurse Practitioner license.
  • Washington Telehealth certification.

PHYSICAL REQUIREMENTS

  • Physical ability to sit, walk, and/or stand for prolonged periods of time.
  • Ability to stand, stoop, bend, grasp, and/or hold work located at the office or other locations as needed.

· Prolonged periods of sitting at a desk and working on a computer.

· Must be able to access and navigate each department at the agency’s facilities.

Job Type: Part-time

Pay: $135,000.00 - $140,000.00 per year
