Job Summary

The Oncology Nurse Navigator (ONN) is a registered nurse that provides personalized, patient and family centered care by eliminating identified barriers to facilitate patient flow through-out the healthcare continuum and help to coordinate patients within Pomerene and outside facilities. The ONN serves as the intermediary and point of contact for patients, caregivers, families, referring physicians and community members. In addition, the ONN will work to facilitate community education, screening and prevention events and work collaboratively within the Navigation Services and Community Outreach Team.

Essential Functions

• Coordinates, organizes, implements, and maintains Pomerene Hospital’s community outreach program.
• Promotes wellness programs and screenings to meet the needs of the community.
• Works closely with Amish Advocate to meet organizational strategies and goals for Amish Services.
• Educates and coordinates care regarding patient’s diagnosis, treatment options, course of treatment, and resources available.
• Works with health care team to ensure safe handoff, coordination of care between facilities as well as inpatient to outpatient or outpatient to inpatient coordination of care. Works with case management to reduce re-admissions.
• Coordinates appointments including all aspects of the multi-disciplinary team (physicians, consults, supportive care services, etc. and accompanies patients as needed to appointments. Ensures that medication adherence issues are addressed.
• Works with Care Team to identify at-risk patients and assists in facilitating appropriate screening processes.
• Trends data and outcomes as established for the navigation program. Identifies gaps to improve patient care across the continuum.
• Performs other duties as assigned or required.

Education/Qualification requirements

The successful candidate will have a valid Ohio RN license, BSN required, with at least 2 years of acute care clinical experience

Must exhibit cultural competence and a passion for serving the community, be flexible, organized, have great interpersonal, communication and prioritization skills

Must be willing to travel as needed and exhibit "I am Pomerene" values

BLS Certification, ACLS (or willing to obtain) - Required

Ability to speak fluent Pennsylvania Dutch and clinical Oncology experience a plus
