Marquis Certified Home Care
Marquis Certified Home Care (CHHA) is in search of a Licensed Practical Nurse (LPN) to care for our members/clients/patients residing in an assisted living facility in Troy, NY for Home Care. Qualified candidates will have a NY LPN license; will have a minimum of 1 year of Home Health, Acute Care, Rehab, or Medical-Surgical nursing experience - CHHA/LTHHCP experience preferred.
LICENSED PRACTICAL NURSE:10 hour shifts, Rotating Schedule.
Location: Troy, NY
LICENSED PRACTICAL NURSE COMPENSATION: $28 per hour
What you will do:
- Provide quality skilled nursing care to residents, care may include but is not limited to those with diabetes, wounds, and injectable medications.
- Document accurately and efficiently in EMR.
- Work collaboratively with the RN of both the CHHA and the RN in the facility as well as communicating with other staff as needed including administrators, providers, and therapists.
- Reporting changes in condition and other findings as per agency protocol.
- Participate in agency's quality assurance program.
- Other tasks as assigned.
Ideal Candidate:
- Active NYS Licensed Practical Nurse Licensure/Registration
- Active NYS Driver’ License, CPR/BLS Card
- One year of Licensed Practical Nurse experience is preferred, however, new graduates are welcome to apply
- Previous homecare experience and/or acute care or rehab experience is a plus
