Come join our team! Pleasant Hill Post Acute is a 50 bed Skilled Nursing Facility located in Pleasant Hill directly off the 680. We are focused on providing high quality care for our patients.

We are looking for a dedicated Social Services Coordinator to join our team. The Social Services Coordinator will be responsible for managing and supervising programs that support individuals with behavioral health needs and as well as discharge planning. The ideal candidate will have a background in social work, program management, and relationship management.

Responsibilities:

- Oversee and manage programs for individuals with developmental and behavioral disabilities
- Coordinate discharge planning for patients back to the community
- Coordinate services with external agencies and community resources
- Monitor program budgets and ensure financial accountability
- Provide administrative support for program operation

Skills:

- Experience in behavioral health or social work settings
- Ability to work with individuals with developmental disabilities
- Strong supervisory and leadership skills
- Proficiency in administrative tasks such as budgeting and program management
- Demonstrated ability in program development and relationship management
