Registered Nurse - Home Health Overview: We are seeking a compassionate and dedicated Registered Nurse to join our Home Health team. As a Home Health Nurse, you will provide skilled nursing care to patients in the comfort of their own homes. Responsibilities: - Conduct comprehensive assessments of patients' health status - Develop and implement individualized care plans - Administer medications and treatments as prescribed by physicians - Monitor and evaluate patients' progress and update care plans accordingly - Provide patient education on disease management, medication administration, and self-care techniques - Perform venipuncture, spinal taps, and other necessary procedures - Assist with memory care for patients with cognitive impairments - Collaborate with interdisciplinary healthcare team members to ensure coordinated and effective patient care - Provide pain management interventions as needed - Phlebotomize and process specimens for laboratory testing - Utilize medical terminology accurately in documentation and communication - Coordinate medical imaging appointments and follow-up care Requirements: - Valid Registered Nurse (RN) license in the state of - Minimum of 2 years of nursing experience, preferably in home health or a related field - Excellent clinical skills and knowledge of evidence-based practices - Strong communication and interpersonal skills to effectively interact with patients, families, and healthcare team members - Ability to work independently and prioritize tasks in a fast-paced environment - Proficient in electronic medical record (EMR) systems - Reliable transportation for home visits We offer competitive compensation, flexible scheduling options, and opportunities for professional growth.
