Job Title: Registered Nurse - Intermediate Care

Job Overview:
As a Registered Nurse specializing in Intermediate Care, you will play a vital role in the healthcare team by providing comprehensive nursing care to patients with a range of medical conditions that require intermediate-level monitoring and intervention. Your focus will be on ensuring the well-being of patients who do not require intensive care but still need specialized attention and support.

Key Responsibilities:

• Patient Assessment and Monitoring:
• Conduct thorough assessments of patients' medical conditions and continuously monitor vital signs and response to treatments.
• Administer medications, treatments, and interventions as prescribed, ensuring adherence to protocols and standards.
• Collaborate with the healthcare team to adjust care plans based on patient progress and changing conditions.
• Care Planning and Implementation:
• Develop and implement individualized care plans based on patient needs and medical diagnoses.
• Coordinate and provide nursing care, including wound care, medication management, and other treatments.
• Ensure compliance with infection control protocols and maintain a clean and safe patient environment.
• Patient and Family Education:
• Educate patients and their families on the nature of their medical conditions, treatment plans, and self-care measures.
• Provide guidance on lifestyle modifications, medication management, and follow-up care.
• Address questions and concerns, fostering open communication with patients and their families.
• Collaboration with Healthcare Team:
• Work closely with physicians, nurse practitioners, and other healthcare professionals to optimize patient care outcomes.
• Participate in multidisciplinary rounds, sharing insights and contributing to care planning discussions.
• Serve as an advocate for patients, ensuring their needs are addressed and preferences are considered in the care process.
• Documentation and Compliance:
• Maintain accurate and up-to-date patient records, documenting assessments, interventions, and outcomes.
• Adhere to regulatory standards, institutional policies, and quality assurance measures.
• Participate in continuous quality improvement initiatives and contribute to data collection efforts.
