Social Workers are the leaders of an interdisciplinary team providing case management to a caseload of migrants, asylees, or homeless individuals with medical, behavioral, or social issues or disorders, based at hotels in the Buffalo area. They advocate for and support the client, engage with community agencies/healthcare providers, and others on their behalf to ensure access to services needed to increase wellness self-management and reduce emergency room visits and/or hospitalizations. They provide culturally competent short‐term counseling, crisis intervention, risk assessment, safety planning, and psycho-education, including:

• Crisis Intervention
• Ability to identify need, as well as make referrals, for outpatient/inpatient care and hospital transport
• Trauma Triage/Trauma-informed assessment
• Peer support
• Conducting emergency evaluation/assessment for: mental health, occupational needs and/or domestic violence
• Advocating on behalf of the family when working with local non-profit agencies or other preventative services.
• Making referrals and appropriate linkages.
• Communicating and collaborating with service providers to enhance engagement and ensure quality.
• Providing social service benefits and other resource information to residents.
• Facilitating family meetings to discuss planning efforts and progress
• Coordinating with emergency services/hospital social workers/discharges, especially around laboring moms & newborns
• Coordinating on domestic violence cases, particularly if a 911 call needs to be made, and providing additional support as needed

Requirements:

• MSW/MHC/MFT Degree or higher.

Experience:

• New Grads Welcome!
• Excellent interpersonal and time management skills required.

Preferred:

• Bilingual in Spanish, Haitian Creole, Georgian, Russian, Wolof, Arabic, Hindu, etc., a HUGE Plus
