Placement Partners MN, Inc. was awarded a CADI case management contract in Hennepin County and we are looking for someone with passion and the desire to assist in further developing and growing this program. You will assist in creating required documentation, program policies, processes, and procedures. You will also be responsible for hiring, training, and supervising case managers for the program. Job Summary

Implements all requirements for licensure and regulatory compliance, program goals and objectives, program services, and program policies, procedures, and practices under the guidance of the Case Management Services Director. Oversees and ensures compliance with all program regulations, practices, and procedures for assigned teams. Hires, trains, and supervises a team of case managers. Manages resources to affect positive outcomes and financial restraints.

Qualifications:

• Bachelor’s degree with a major in social work, psychology, sociology, or a closely related field
• Bachelor’s degree with a major in any field and one year of experience as a social worker/case manager/care coordinator
• Previous Case Management Experience in Hennepin County is preferred, but not required
• Valid Driver’s License with current auto insurance and has an acceptable motor vehicle record
